What is a 12 Lead ECG?

Configuration of the 12 Leads

The 12-lead ECG consists of six limb leads and six chest leads, derived from only 10 electrodes. The limb leads include Lead I (left arm to right arm), Lead II (left leg to right arm), Lead III (left leg to left arm), aVR (right arm), aVL (left arm), and aVF (left leg). The chest leads, also known as precordial leads, are labeled V1 to V6 and are positioned as follows:

  • V1: Right sternal border in the fourth intercostal space.
  • V2: Left sternal border in the fourth intercostal space.
  • V3: Midway between V2 and V4.
  • V4: Clavicular line at the fifth intercostal space.
  • V5: Anterior axillary line at the same horizontal level as V4.
  • V6: Midaxillary line at the same horizontal level as V4.
